Prevention is the ‘action of stopping something from happening or arising’. The Lower Hume PCP coordinates and assists agencies to undertake work to ‘prevent’ disease or chronic illnesses within our communities. Together with our member agencies we collaboratively created and are implementing the Lower Hume Strategic Prevention Plan 2017-2021, with our goal that ‘Lower Hume communities support healthy eating and physical activity.’

The Lower Hume Strategic Prevention Plan 2017-2021 builds on the Collaborative’s achievements from the last four year plan (see achievements below). In partnership with funded and non-funded partners, local government and community organisations, our objective is to Increase the community’s capacity to design and implement local solutions to create environments that support healthy eating and physical activity choices.
To achieve our objective we will be rolling out the RESPOND project throughout Lower Hume, aiming to engage the whole community in the identification of enablers and barriers to healthy eating and physical activity, how these factors interact within the complex community settings or “system”, and develop community led solutions that promote healthy eating and physical activity behaviours.
